DEATH REGISTRATION – CURRENT REGISTRATION IS WITHIN 60 days
(in case of late registration a requesting affidavit is required)
- Death occurred at Health Facility
- Certification of cause of death (BMD 10).
- Deceased’s Birth Certificate
- Identity card of the deceased.
- Identity card of the person registering the death.
- A document (i.e. a Birth Certificate) that links the deceased with the informant.
2. Death occurred at Home
- Letter from Umphakatsi, with official stamp, where deceased was resident
- Deceased’s Birth Certificate
- Identity card of the deceased.
- Identity card of the person registering the death.
- A document (i.e. a Birth Certificate) that links the deceased with the informant
2. Death has not occurred either at home or hospital
- Police Report
- Letter from umphakatsi acknowledging the death
- Certificate can be registered without post mortem results
External Death Registration
- Certification of cause of death issued by a doctor in the country where the deceased passed away.
- Deceased should be Swazi in terms of the Constitution
- Birth certificate and ID card of deceased
- Identity card of the person registering the death.
- A document (i.e. a Birth Certificate) that links the deceased with the informant.
- Deceased’s passport copy certified copy of passport page with particulars, photo and entry stamp
- (REGISTRATION ONLY DONE AT HEAD QUARTERS)
